For valuable consideration received, including the opportunity to be included in materials distributed by Best Singing Telegrams, the undersigned hereby grants irrevocable permission for Best Singing Telegrams to use, copy, modify and publish the [video/photograph/image] attached to this form and his/her likeness or image, as well as the image or likeness of anyone else, appearing in the [video/photograph/image] in any format for any purpose (the “Work”). The undersigned agrees that he/she has obtained permission from all other persons appearing in the Work to grant this release on their behalf.

The undersigned understands that the Work may be used in marketing materials, including but not limited to on websites and in social media. The undersigned further understands that any use of the Work is without payment to the undersigned, his/her heirs or assigns and that SAC is under no obligation to use or publish the Work.

The undersigned waives any right to inspect and approve the finished product that may use the Work or to which the Work may be applied now and/or in the future, whether that use is known to undersigned, his/her heirs or assigns.

The undersigned hereby holds the Company and its present, former, and future officers, directors, employees, agents, and assigns (“Indemnified Parties”) harmless and releases the Indemnified Parties from any and all claims related in any way to the Work, including, without limitation, any and all compensation, publicity, copyright, and/or privacy claims.

The undersigned certifies that he/she is at least 18 years of age (or if under 18 years of age, that he/she is joined herein by his/her parent or legal guardian) and that this release is signed voluntarily, under no duress, and without expectation of compensation in any form now or in the future.
